The Transition Coordinator is responsible for completing: intakes (from referrals); assessment of eligibility and appropriateness for transition into the community; developing a plan for transition for moving into the community with the client/consumer; and follow-up after placement to ensure continuity of care.

Required Minimum Qualifications

Master's degree.

Additional Required Department Minimum Qualifications:

Valid U.S. driver's license. Must be at least 21 years of age and have an acceptable motor vehicle report as determined by the insurance carrier.

Preferred Qualifications:

Master's degree in Rehabilitation Counseling or related field is preferred.

Pay Grade/Pay Range:

57 Monthly (exempt): - Minimum - $ 31969.56 Midpoint - $ 46342.44 Department/Organization:212601 – Ed Studies Psy/Res Method/Counsel Positions are available in Mobile, Montgomery, Talladega, Gadsden and Florence.
