Under limited direction, increase patient access to quality health care by providing nursing expertise in diagnosis and treatment to prevent, remediate or alleviate illness and promote health. Consults and collaborates with all members of the healthcare team, and provides training on Rapid Geriatric Assessments to community physicians and interdisciplinary groups of students.

Minimum Qualifications

Master’s degree in nursing, Missouri RN license, and certification as an Advanced Practice Nurse required.

Job Duties and Responsibilities

Provides patient care, conduct physical exams, complete health history summary, document patients’ progress, assist with patient care management. Provides training and education opportunities to community physicians, interdisciplinary teams and their students on rapid geriatric assessment.

Compiles and enters clinical data into database; reviews and assesses data collected; collaborates with investigator and statistical consultants; submits documentation and correspondence for compliance with protocols.

Keeps informs by attending QI meetings, documents progress in medical record charts and interacts with home health agencies.

Performs other duties as assigned.
