Under general direction, provides social work and counseling services to patients, families; plans, implements, and evaluates delivery of healthcare; participates in learning and educational programs.

Graduate from an approved school of social work; supplemented with one (1) year of previous work related experience in a healthcare environment. State license or LCSW required.
Experience in clinical counseling and family-centered case management required as will be an active part of position. Experience and/or exposure to motivational interviewing preferred. Experience in postpartum depression counseling is preferred.
Performs and documents patient assessments, including physical, psychosocial, spiritual and cultural needs; establishes and coordinates an interdisciplinary plan of care from patient’s assessment; identifies patient’s needs across the health care continuum.
Identifies, initiates and documents individualized social work intervention plans; provides ongoing collaboration with appropriate health care members for coordination of patient care plans; evaluates effectiveness of social work plan and modifies that plan as patients progress toward expected outcomes.
Participates in Danis Pediatric Center committee memberships, staff meetings and educational programs, including in-services as required to meet the needs and objectives of sponsored research projects that Social Worker is working on; may participate in development, implementation and use of educational material to improve understanding of medical illnesses as related to services involving the P-Square Parenting Clinic and the Happy Mothers, Healthy Families Program to the clients Social Worker assesses on sponsored research programs. Obtains and maintains certifications and licenses necessary for practice and sponsored research projects; may attend lectures or complete course work to advance professional knowledge in order to meet clinical requirements of the sponsored research programs that Social Worker is directly working on.
